You only need a broken sheet and painting, a lot of red painting.
And of course, go out at the night of Halloween to ask for Trick or Treating or to scare people.
About the theme of the costume, well, i have already said before, it is about a insane man scaped from asylus, and very injured due to violent treatment, both mentally and physically. A classic theme of many horror videogames, and one of the best.
